After you have completed the form, please send it by fax to (949) 857-4611 or email it to perio@millerandbounds.com. The security and privacy of patient data is one of our primary concerns and we have taken every precaution to protect it.
At Miller & Bounds we strive to care for your patients as you would. The referral partnership is vital to the successful outcome of any complex treatment, and it is our goal to communicate with you clearly and effectively during all phases of treatment. Through mutual collaboration the patient gets the best qualities of all practitioners involved.

For Your Consideration: Dr. Bounds is always available to be an educational resource for your office – We offer seminars, lunch and learns for both periodontal and dental implant-based reviews, implementation of periodontal maintenance programs and observation of surgeries in our office. Our Professional Relationship Director, Erinn Igarashi is also available to visit your office to conduct trainings and seminars specifically designed for the advancement of your practice and to help improve communication and collaboration for your team members. Our office will advocate for your practice and your treatment plans – We know that our practice exists due to continued support and referrals based on the trust you place in us – and we will respect that by supporting recommendations and decisions made by you and your staff. We always strive to appoint patients back to the referring office for continued dental needs and treatment. We strive for excellent and complete communication both with patients and your office – This will help minimize any confusion between patients and each office. We can use whatever means of communication you prefer: phone, text, fax, email or mail.

Tips for Referring a Patient

• If possible, please appoint the patient for a consultation with our office while they are still in your office. This way we can get your patient the earliest and most convenient appointment time possible.

• When the patient leaves your office, please email the referral information to our office at perio@millerandbounds.com.

• When emailing the referral slip, please include the radiographs and insurance information for the referred patient. We prefer to have an FMX so we can diagnose as much as possible, even if you are sending the patient over for a limited exam.

• If you do not have digital radiograph capabilities, please mail us a copy of the radiographs to:
